SHIPPING FORM
  • Print our packing slip or write your name, address, and phone number separately and include it in your package.
  • Pack your material securely in a box, freezer bag or secure envelope. Insert the package and contact information in the FedEx box or USPS box making sure to include enough packing materials (like bubble wrap or newspaper) so the contents will not move.
  • Seal the box and tape any seams. Registered mail requires the use of paper tape and not plastic tape.
  • Insure your package so it is safe and tracked.
